Cognitive Consultation

Initial consultations are typically an hour and a half. They include a thorough review of your symptoms and concerns, medical history, previous treatments, goals, a neurologic exam, and cognitive/neuropsychiatric assessments, as indicated. The goal is to provide an opinion on your diagnosis and make recommendations for treatment. In some cases, a second consult visit may be necessary.

A consult may be the beginning of ongoing treatment with NWCCNH or may be a one-time service care being provided by another clinician.

Following a consult, ongoing care may be provided by the referring clinician or another member of a patient’s treatment team. Alternatively, ongoing care can be provided by NWCCNH if the consult results support this. Appointment frequency is determined on a unique schedule based on individual needs. Follow up visits are typically 40 minutes.

Medicare Guide Program

NWCCNH has been selected by Medicare to participate in the ​the Guiding an Improved Dementia Experience (GUIDE) Model beginning in July 2025.

Comprehensive Cognitive Care Planning Visits

A visit to create a personalized guide to support individuals living with memory loss or dementia. It helps patients and their families manage medical care, stay safe at home, and connect with the right support and resources.

The process starts after an initial consultation and is a separate visit. Medical care is tailored to each person, with a focus on managing symptoms, reviewing medications, and planning ahead for future decisions.

We assess daily living skills, home safety, mood and behavioral changes, and driving ability. If support is needed, we recommend strategies and tools to help the individual remain as independent as possible.

Care partners are given education, emotional support, and referrals to helpful services like support groups and respite care. We also connect families with community resources—such as adult day programs, transportation, and legal planning services.

The plan is updated over time through regular follow-ups, ensuring care continues to meet the person’s changing needs. A cognitive care plan helps families feel more confident and supported through each stage of the journey.
